Student Staff / Faculty
  • An official picture ID
    (driver's license, passport, or state ID)

Employee IDs will be issued at New Employee Onboarding Orientation.

You may request a replacement card here or login to the employee portal to submit the online form.

  1. The request must have a Campus Public Safety Incident Report; and
  2. Receipt of payment ($25) for lost MDID/Access Card (Paid at the Bursar Office).
