MDC's Volunteer Income Tax Assistance (VITA) Program offers free help for Students, their families and the public in general.

FREE SERVICE designed to help prepare Individual Income Tax returns for community residents that earn $67,000 or less.

Items You Need to Bring

  • Physical Photo identification (i.e., driver's license)
  • physical Social Security Card (original) or ITIN for taxpayers, spouse, and dependents
  • Birth Date for Primary, secondary, and dependent on the tax return (Birth Certificates)
  • IP PIN letter issued in January 2025
  • Health Insurance Marketplace Statement (1095-A)
  • Wage and earnings statement(s) From W2, W2G, 1099R, from all employers
  • Unemployment Compensation 1099G
  • Social Security Benefit Statement SSA-1099
  • 1098T - Tuition Statement
  • Interest and dividend statements from bank (forms 1099)
  • A copy of last year's Federal and State return, if available.
  • Total Paid for Day Care provider’s and identifying number
  • Bank routing numbers and Account Numbers for Direct Deposit (bring voided blank check)
  • Pension withdrawal form 1099
  • Bank documents with mortgage interest and property tax information
  • Schedule C income and expenses reports
  • Any letter received from IRS
  • To file Taxes electronically on a married filing joint. tax return, both spouses must be present to sign the required forms.
  • Other relevant information about income and expenses
